RN CASE MANAGEMENT

RN Case Managers are critical in helping people avoid hospital readmissions and secure an improved quality of life.  Our model incorporates evidence-based best practices that support each client's specific personal health and wellness goals.  Our understanding of health is that it includes physical, mental emotional and spiritual care.  Nurse case managers provide support in patient advocacy, as well as guiding clients and their families through major transitions.

Clients benefit from one-on-one consultations with our Registered Nurses who are dedicated to understanding the client's specific health concerns. If appropriate, the RN will consult with client's primary provider to create a comprehensive care plan, which might include scheduled assessments, care coordination with other care team members, health education and very personalized interventions.  Our case managers are constantly evaluating process improvement, quality measures and patient outcomes to improve our care.

Other care options include: home safety evaluations, cognitive and emotional assessments, resource coordination, care navigation, guardianship care and on-going education.

The RN Case Manager has close oversight with care team Aides and provides weekly appointments to follow up on care plan.
