Clients benefit from one-on-one consultations with our Registered Nurses who are dedicated to understanding the client's specific health concerns. If appropriate, the RN will consult with client's primary provider to create a comprehensive care plan, which might include scheduled assessments, care coordination with other care team members, health education and personalized interventions.
